Procedure for requesting financial assistance

  • The assistance can be requested by any madhwa individual for self or acquaintance

    Step 1

  • The request for assistance needs to be formally submitted in the prescribed form by the beneficiary or his/her close relative or acquaintance.

    Step 2

  • The request needs to be categorised based on its purpose such as medical, education, sustenance, along with brief information regarding the situation.

    Step 3

  • The personal details like name, mobile number, bank details etc of beneficiary need to be provided in the form.

    Step 4

  • Needs to provide names and contact details of 2 references.

    Step 5

  • The copy of signed form needs to be submitted by email to jssmadhwa@gmail.com

    Step 6

  • JSS committee will review the request in conjunction with reference check.

    Step 7

  • JSS Committee will decide the details regarding disbursement of assistance and communicate the same to requestor.

    Step 8

  • The decision by JSS committee members regarding the request will be final.
